Fresenius Health Partners PPO SNP is a Medicare-approved PPO SNP plan, available exclusively to people with ESRD. Fresenius Health Partners operates as a Chronic Care Special Needs Plan. The plan is offered through the following organization that contracts with the Federal government: Sterling Life Insurance, a Coordinated Care plan with a Medicare Advantage contract and a Medicare-approved Part D sponsor.
