Fresenius Health Partners (PPO SNP) is a Medicare-approved Medicare Advantage Preferred Provider Organization plan open exclusively to people with ESRD. Fresenius Health Partners (PPO SNP) operates as a Special Needs Plan for ESRD patients with a range of benefits and services that are designed for people on dialysis. Here are some of the advantages of Fresenius Health Partners for you as a physician:

• Fresenius Health Partners pays the equivalent of Medicare allowable, less enrollee cost share.
• This is not an HMO, but a (PPO SNP) Medicare Advantage plan.
• There are no capitated payments. You are paid for the services you provide on a fee-for-service basis.
• There are no gatekeepers and no referrals required for non-ESRD related services; you and your patient determine the course of treatment without interference.
• Fresenius Health Partners adheres to Medicare's Prompt Payment Requirements with 95% of all clean claims paid within 30 days of receipt.

If you do not wish to join our network but are willing to see people who are enrolled in Fresenius Health Partners (PPO SNP), please tell the patient you are not a network provider but are willing to provide care for them. If you agree to provide care you will be expected to submit your claims to Fresenius Health Partners and not to Medicare. You also agree to accept our fee schedule, Medicare rate less member cost sharing, and do not balance bill beyond the member cost sharing amounts. Please note that Fresenius Health Partners has a 90-day timely filing limit from date of service. A clean claim must be received by Fresenius Health Partners before the 90 days have expired. A clean claim means a claim that has no defect, impropriety, or lack of any required substantiating documentation.
