Home Health Monitoring Program

The Home Health Monitoring Program is composed of two resources that work together to give you the best care.

Personal Nurse Care Manager
Your Nurse Care Manager will call you regularly and keep a personal preventive plan of your ongoing health. Your Nurse Care Manager is there to offer support, coordination, education, and advice to improve your total health care experience.

Home Health Monitoring Device
You will receive a Home Health Monitoring Device to use in your home. It is an easy-to-use system that communicates with your Nurse Care Manager and alerts your nurse on any change in your health. The system includes a blood pressure cuff allowing you to check your blood pressure daily as well as a digital scale to better track your weight. Additionally, the device connects with your blood sugar glucometer to help us help you better manage your diabetes. The information is tracked and analyzed by your health care team.

Medical/Nutritional Meals

Good nutrition is important in helping you stay healthy. It may be challenging to eat a well-balanced meal for a variety of reasons. Your nutrition could be affected because of your health condition. To help you maintain a proper nutritional level, you may receive meals, and protein or protein-calorie supplements. These are formulated to help you stay well. This benefit has no cost to you and is shipped directly to you at your home if there is a medical or nutritional need.

Education and Empowerment

You will receive focused educational materials each month, which your Nurse Care Manager will review with you. You will also receive a quarterly newsletter with informative articles and kidney-friendly recipes. Additionally, you receive a monthly health status report from your Nurse Care Manager.

Diabetes Support

Keeping track of your blood sugar level is essential in maintaining good health. Members who have diabetes will receive a glucometer and monthly testing supplies, with no out-of-pocket expense, shipped directly to your home. You may test as often as you want in order to manage your blood sugar appropriately.

Preventive Coverage

Members have up to $400 per year to use for routine dental services (including extractions). Vision coverage includes $0 co-pay for eye exam, including one pair of eye lenses, and $125 for frames and contact lenses (in network), or plan pays up to $50 for eye exam and $125 for eyeglass lenses, frames and contacts lenses (out of network). Routine vision exam and eyewear are not normally covered by Medicare.

Medication Therapy Management (MTM) Program

A Medication Therapy Management (MTM) Program is a free service we may offer. You may be invited to participate in a program designed for your specific health and pharmacy needs. You may decide not to participate but it is recommended that you take full advantage of this covered service if you are selected.
