Fresenius Health Partners (PPO SNP) (Preferred Provider Organization) is a Medicare Advantage Plan exclusively for individuals with ESRD on dialysis. The benefits and services are designed to help you, as a dialysis patient, to better manage your health care needs. You will want to take full advantage of these benefits which are highlighted below.

Fresenius Health Partners (PPO SNP) is a subsidiary of Fresenius Medical Care Health Plan (FMCHP). Fresenius Medical Care is a large renal provider and has access to outstanding programs like vascular access surveillance, nutrition programs, pharmacy services, and renal-specific laboratory services.

Because of these rich resources, we are able to offer these unique benefits:

KidneyTel® Program

The KidneyTel program is comprised of two resources that work together to give you the best care.

Personal KidneyTel Nurse
Your KidneyTel Nurse will call you regularly and keep a personal preventive plan of your ongoing health. Your nurse is there to offer support, coordination, education, and advice to improve your total health care experience.

Home TeleHealth Monitoring System
You will receive a KidneyTel Home TeleHealth Monitoring System to use in your home. It is an easy-to-use system that communicates with your KidneyTel nurse and alerts your nurse on any change in your health. The system includes a blood pressure cuff allowing you to check your blood pressure daily as well as a digital scale to better track your weight. Additionally the device connects with your blood sugar glutometer to help us help you better manage your diabetes. The information is tracked and analyzed by your health care team.

Nutritional Support

Complimentary renal vitamins from Nephro-Tech and oral nutritional supplements, such as Ensure or Glucerna, are delivered to your home if there is a medical or nutritional need.

Education and Empowerment

You will receive focused educational materials each month, which your KidneyTel nurse will review with you. You will also receive a quarterly newsletter with informative articles and kidney-friendly recipes. Additionally, you receive a monthly health status report from your nurse care manager.

Diabetic Support

Diabetic members will receive a free glucometer, glucose strips, and testing supplies.

We will pay a total of $600 for any combination of vision and dental.

Medication Therapy Management (MTM) Program

A Medication Therapy Management (MTM) Program is a free service we may offer. You may be invited to participate in a program designed for your specific health and pharmacy needs. You may decide not to participate but it is recommended that you take full advantage of this covered service if you are selected.

Benefits, formulary, pharmacy network, premium and/or co-payments/co-insurance may change on January 1, 2013. Members may enroll in the plan only during specific times of the year. Contact the plan for more information.

Individuals must have both Part A and Part B to enroll. You must continue to pay your Medicare Part B premium.

Members must use network pharmacies to access the prescription drug benefit, except in non-routine circumstances. Quantity limitations and restrictions may apply to some drugs.

You may be able to get Extra Help to pay for your prescription drug premiums and costs. To see if you qualify for Extra Help, call: 1-800-MEDICARE (1-800-633-4227). TTY users should call 1-877-486- 2048, 24 hours a day/ 7 days a week; the Social Security Office at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY users should call, 1-800-325-0778; or Your Medicaid Office.

People with limited incomes may qualify for Extra Help to pay for their prescription drug costs. If you qualify, Medicare could pay for up to seventy-five (75) percent or more of your drug costs including monthly prescription drug premiums, annual deductibles, and co-insurance. Additionally, those who qualify will not be subject to the coverage gap or a late enrollment penalty. Many people are eligible for these savings and don't even know it. For more information about this Extra Help, contact your local Social Security office or call 1-800-MEDICARE (1-800-633-4227), 24 hours per day, 7 days per week. TTY users should call 1-877-486-2048.

You must also use plan providers except in emergency or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-network providers neither Medicare nor Fresenius will be responsible for the costs.
